GI
Esophagus, pancreas, liver SBRT, rectum and anal canal — setup, motion management, image fusion, target volumes, abdominal and pelvic OARs and cited constraints.
GI planning spans two very different regions. In the upper abdomen — esophagus, pancreas and liver — the targets move with breathing and sit against the stomach, duodenum and bowel, so motion management, fasting and careful image fusion come first. In the pelvis — rectum and anal canal — the targets are long and concave, wrapping around the bladder and small bowel, and the bladder and bowel change shape from day to day.
Preoperative and definitive esophageal chemoradiation — setup, 4DCT for junctional tumors, PET/CT fusion, longitudinal CTV, and cited lung, heart, liver and kidney limits.
Conventionally fractionated pancreas chemoradiation — fasting, contrast timing, breath hold and fiducials, why image fusion is hard here, and cited duodenum, bowel, stomach and kidney limits.
Stereotactic treatment of hepatocellular carcinoma and liver metastases — motion management, fiducials, multiphase contrast CT, MRI fusion, and cited 3- and 5-fraction organ limits.
Long-course and short-course rectal treatment — prone or supine setup, MRI fusion, mesorectal and nodal volumes, sequential boost or SIB, and cited pelvic constraints.
Dose-painted IMRT for anal cancer — frog-leg setup, markers and bolus, MRI and PET fusion, primary and nodal volumes at several dose levels, and RTOG 0529-based organ goals.
